To download contracting paperwork, please click on the appropriate link after the company name for the type of contracting you desire (Agent or Corporate). Some companies combine all forms into one package as indicated by "Combined".

If you have previously contracted with the company you are seeking to be appointed with under PFG Marketing Group, you should check with the carrier to see if you are eligible to transfer. If not, you will need to secure a written release from your existing upline. If you prefer, you may contact our office and we will call to check your transfer eligibility for you.

CONTRACTING INSTRUCTIONS:

1.  Print out the entire document

2.  Complete all areas requested

3.  Scan, Fax or Mail the completed forms along with copies of your Licenses,
     Proof  of  E&O, Voided Check, AML Training Certificates and any required
     written explanations and documentation to:
